About the Hub

The Evolve Hub is truly here for you, the community. Whether you're unsure where to start your mental health journey or you know someone who is in need of support, if you need advice on available services, or require a direct referral to the right kind of support, we're here to help you navigate the system.

Here at Evolve, our core mission is simple: to make mental health care more accessible to everyone in the Hunter Region. We understand that navigating the world of mental health support can sometimes feel overwhelming. Knowing where to turn, who to trust, and what options are available shouldn't be a barrier to getting the help you need.

We've created a dedicated space – both physically, here at 54 Ridley Street, Charlestown, and through our outreach supports, services and events – to be your first point of contact for information, advice, and referral regarding mental health and wellbeing.

To make this vision a reality, we're proud to partner with a fantastic network of dedicated professionals, organisations and community members to support the work that we do.

If you need to connect to care or are feeling overwhelmed trying to find the right support, please reach out or visit us at 54 Ridley Street, Charlestown. We're committed to making access to mental health support easier, more understandable, and ultimately, more effective for everyone in the Hunter Region.

We're here to listen, guide, and connect you with the support you deserve.

Information, Advice &
Referral

Our information. advice and referral service is open to everyone across the Hunter region via telephone (02 4096 1100), email (admin@LMNSPN.org.au), or by coming in to the Hub (54 Ridley St, Charlestown). We offer a free service to connect you to the support and information you need to navigate your mental health journey. This program is called 'Evolve'.

The Evolve program will connect you with one of our trained support staff who will discuss with you what your needs, challenges and barriers are and what you want to achieve. From here we will recommend services and supports that meet your individual need and assist you to connect with these supports. But Evolve doesn't end there. We will provide follow up care to ensure these referrals are working for you, and to assist you along a Pathways Plan, that assists you to progress from one support to the next, towards your overall goals. We are here to provide a continuum of supports that help you to achieve the results that you are seeking.

We are here to guide you every step of the way.
